One thing that happens to me is the server suddenly lags registering me as right next to a mob and thus i take constant dmg unable to heal and i die
this once tped me to Bucie from near Cassiop

Were you playing 1.9? quite often dismounting in 1.9 teleports you back hundreds of blocks

Also I'm amazed the vast majority of people don't use /class to avoid fire damage. It's time consuming, but not as much as dying

 

Doing /class resets the loot chest timer, and lets be honest here, you're only running around molten heights becasue you're on a loot run.

I die most oftan to Lag, Fire, And wandering into High lv places (I just got to gavel and Almost wandered into Olex,)
