I'd guess that he had some highly dark, highly powerful relik, and sacrificed most of his useless creations with either some obscure ritual or one of his own creation to create a rift between realms, essentially creating a portal. I wasn't able to find an actual weapon in the game, but we can assume he probably used a relik, since shamans (in-game) use them.
Pretty sure Dullahan was executed AP because in the Headless hunt quest they say he was executed hundreds of years ago and the portal was opened 1000 years ago. Now while its true that a thousand years ago is still a few hundred, his prisoners also say they have been trapped for centuries. Although it is also said that the decay appeared after he was killed...
Not sure but I'm pretty sure he tried to open his own portal to his own dimension and instead went to Dern because he says something about a ''dark god''
Also you said that Bak'al is a corrupted entity. To some degree that's true, in but Fate of the Fallen Theorick states that the last human to enter the portal became Bak'al, meaning that Bak'al is a corrupted human.
