These things are pretty minor, but caught my attention early on. If you have 2 soul points and you die, you lose all your items. This doesn't make sense, because you'd assume you use those soul points to keep your items. It's quite weird that you use 2 soul points and they basically do nothing, because you're going to lose your items anyways

Also, in the missions, when the timer hits 3:59, it says that I have 3 minutes left, which could also be confusing. Yes, I'm just nitpicking, but we want to make this game perfect right?

What's confusing about soul points? You start off with 10-15, and when you die you lose 2. When you 0 soul points, you lose all your equipment. You don't lose anything unless you die with 2 or 1 soul point remaining.

Soul points are essentially your soul. In case you didn't notice, soul points regenerate, so it's just a matter of skipping away from that dangerzone.
As for lore-wise, I believe that your soul is like a person itself, and each "death" you take doesn't actually kill you but instead hurts your soul. However, every time you see a new day, the soul becomes hopeful and heals a bit. But when battered and bruised past it's limit, the soul will die and you will die a true death.
