The Mobs Have Custom Heads, Just Wondering About Something

Title says all + How do they all just find a head that fits perfectly onto a mob? Is it that someone volunteers? Or it just happens that someone has a head of Qira or Death?

I'm curious

 

There are multiple ways to do this.

Find MC accounts that have the head skin you want, get their head, and pray they don't change their skin.
Make a head skin you want, and find somebody willing (or buy an mc account) to make it their skin, and then use that head.
In texture packs, it is possible to retexture the default skins (steve and alex).
