Textures Not Working Right

Hey everyone! The title of this might not make too much sense so here is my problem: My player heads and a few of the textures don't work. For example wood looks like a player head (not like a wood player head) i have a few screenshots to prove/show this. I have been playing and it doesn't bother me too much, but I would like to know if there is a fix.

For example, my weapon looks like a weapon, but Nivilan Honeycomb doesn't look like a honey comb which I believe it should, it looks like a very basic player head

 

images doesn't work.

What mods (if) you are using? What version do you play on?

 

I play on 1.9.4 with optifine, I dont use mods (should I?) one image that doesn't look right (it works) is the pouch. On my screen it looks like a diamond axe, when I assume it should look like a pouch

If it doesn't work I have no idea what to do. Make sure to create Bug Report then

 

Can you try using 1.12.2? If nothing changes, possibly try emptying your server resource packs folder and having it get re-downloaded.
