The Phantom Program is a series of assignments in BF4 which are enabled by going to the "Leaderboards" and clicking on the Phantom icon

Clicking it would bring something like this up
Just a little note, you must have Battlefield 4 Premium to unlock the assignments ahead.

You must have completed all of the previous 3 assignments.

Spoiler: How to unlock it

You must wear all assignment rewards in game and you must wear 1 of 4 Final stand dogtags, you must get 3 friends to help you go to the Phantom room, those friends must have to do what you did, except your whole group of 4 must wear all 4 dogtags. A link for dogtag locations plus a better explanation http://imgur.com/a/hzBRN . Go into the elevator located in the back of Hangar 21. At the bottom of the elevator is a room blocked by a forcefield, you must enter a passcode to enter the room.
