Neutral damage basically affects all mobs regardless of elemental defenses/weaknesses the same. Damage of a specific element only has an effect on a mob that either has that element as a weakness or a defense, and doesn't affect mobs without that element as a defense or weakness. For example if I had a weapon that only deals 30 base thunder damage per second (with no neutral damage on the weapon), and I used it against a mob with no elemental defenses or weaknesses, I would deal 30 base damage to the mob. If I used it against a mob with thunder defense, I'd deal less than 30 base damage to the mob, and if I used it against a mob with thunder weakness, I'd deal more than 30 base damage.

It's worth noting that the conversion is kind of weird when using spells.
