    if you exit thanos then go on the path that leads you to the temple of the lost (go past the temple and keep going straight) and past the hobgoblin farmers, you'll eventually find elefolk hunters/soldiers, which have a low chance of dropping the toenail, or a rare mob (the elephelk iirc) that has a high chance (maybe 100%) of dropping the toenail.
