If a crafted item goes down to 0 durability, does the item's stats also go to 0% or does it linger at a set percentage? Ex. 3000 poison boots go down to 0 durability. Does the poison go to 0 poison or like a percentage like 33% and so becomes 1000 poison?

As the durabality is under a certain value, say 30%(just an example), then the stats on it will decrease in effectiveness when at 0 the item becomes unusable
When you repair it back to full durability the stats will be at 100% effectiveness again

From my experience with weaponsmithing, this is not the case. Once a crafted item comes closer to 0 durability, it rapidly drops in effectiveness, however at 0 durability the item will still be usable with only 9% or 10% Effectiveness (meaning effects and damage is divided by 10).

From experience, skill points boosts (Like +5 Intelligence) are not affected by effectiveness either, probably to prevent situations of not reaching the skill point requirements from using damaged Crafted Items. Additionally, Effectiveness only starts dropping at the 50% Durability mark.

Does the base health boost or base damage from armor and weapons also become affected by this effectiveness drop?

Not sure with armor, but I do think base damage does decrease as effectiveness decreases (even though it does not show).

Not sure how this works on Armor but I know that when you get a weapon down to 0 durability and 9% (lowest I think) it just wont deal any damage... like in base damage. The game doesnt show that though.
