Can someone tell me what happened here?

So my cousin was over at my house playing on his character, he comes out to the living room to get me because this happened, I am just trying to figure out what in the hell happened. At first, I thought he might have thrown it out on accident but I reviewed the clip and it didn't look like he did. I am just so confused, also sorry for the terrible quality. :/

i honestly don't have a clue
he was decently high on soul points and no message said he dropped anything upon death
maybe he accidentally pressed the drop key while lagging?

 

he might have hit the offhand key right as he died, if you play it back frame by frame you can see the wand swap to the left side of the screen

 

You can see it dropped on a spruce stair right in front of him(next to the pot) at 3:35(and for the rest of the vid)
