For a first playthrough, I'd probably do the dungeons at least once just to experience them. The caves are wholly optional and imo most shouldn't even be listed in the "recommended" section of the quest book.

There are items of all tiers at the endgame, but the mythic weapons are generally better, so if you have the ability to use them, it's generally advisable for many or most builds. They're common and affordable enough now that most people can, so you'll see them generally seen as the endgame option for each element. It's worth noting there's only 1-2 mythics for each element though, so they may not work for a given build, especially if it's a more unusual or unique build.

I wouldn't do side quests at all generally. They're just grinding, and there's not really any reason to do them if you don't need xp, unless you enjoy the process of grinding or if you're intent on 100%ing the game.

It's not even that they give terrible xp. There's just enough other ways to get xp that grinding for rare materials isn't really worth it from a time standpoint.
