Pretty sure statistically, Azure Blossom is the rarest item. Only being dropped by one mob (in the Realm of Light) with like some crazy low chance of dropping.

 

probably emerald pouch t10 as said above, however Mama Zomble's Memory is also very rare

as for unobtainable items that were once obtainable and are now scarce, there are many but a notable few include:
Salted's Poop
Grian's Trusty Ocean Map
Hallowynn Mask
Trick & Treat
Santa Hat
Santa Pants
Santa Boots
Wynnterfest souvenirs

EDIT: Also, definitely T10 Emerald Pouch. One was found in the beta that introduced them, and as far as I know only one is known to exist on main even now.

Emerald Pouch t10 is the rarest. I'm pretty sure only one has ever been found. The spawn conditions are so ridiculous ontop of the RNG that for a while we genuinely thought it was impossible to find

The "Inhibitor" chestplate is pretty rare, being a rare drop (1/1000 I think) from Qira. Yes, Qira.

For ingredients, azure blossom is usually the one people think of first, but there's also transformativity which only drops from a really rare obscure mob in kander forest. There's also the aforementioned "disturbed dye" ingredient which drops from lost eye, a rare mob that replaces chiropterror in TNA with a 1/30 chance.

Emerald Pouch Tier 10 has and always will be rare. I think only one pouch has legitimately been found on main since emerald pouches were introduced over a year ago.

"Hopeful Lantern" is a rare item/trinket that has a 1/1000 chance of being given to you by the hoarder in void village when giving him voidgloom roses.

And of course, all seasonal/festival items are rare in their own right, although the rarity of most depends on how close or far we are to that season.

I'm only including obtainable things, as all unobtainable ones are going to be extremely rare because a finite amount of them exist with no way of a player getting one without paying up.

I have one of those.

With a crafted/discoverer build and double loot, you can get one every 2 hours average by spamming /class next to where it spawns.

The rarest item is an 8 rolled convergence. It is an obtainable item, but I imagine no one has it since takes 150+ stacks of liquid emerads

 

Rolling a convergence to [8] requires over 584 stacks of le.

The most a single inventory can hold, with 28 Tier 10 pouches, is 112 stacks.

You might have been thinking about [7] but I don't know how since you a said "150+". That costs 116 stx which is still impossible.

 

Can't [7] be rolled on Ironman? Due to the fact that the ironman pouch can hold emeralds as well.

Transformativity is rare? I have at least 3 of those. (I grinded nii runes in Kander.)
