An error has occurred - Ban Appeals

I would like to put my Minecraft name, but an error occurs, I don't know what this error is about, could someone help me?

- I was banned a while ago and I would love to play again, and I would like to try my luck to ask so that they can take this punishment away from me.
It's been over a year since I've been banned, and I really miss playing, even removing everything from my account, and starting over, I would love to, I just want to go back to playing.

Do you know the username you were using when you were banned, if you have changed it since then? You'll probably need to use it and not your current username.
