    What exactly happens to an item when it gets reworked?
    Do they get their stats and rerolls reset?
    Do they just get their stats reset but keep the rerolls?
    Do they get un identified?

    When the stats of an item are just increased or lowered, the game will update your item automatically, same when stats are removed. When a new identification is added, however, a re-identification is required to get that stat on your item.

    How exactly does it update changed stats automatically? Does it set it to max, reroll that stat, or something?

    It's actually automatically applied at a random possible value
