Warrior/Knight Help Apocalypse Build

I want one with good damage output and can stay alive for a while. Please put skill points along with build if possible.

 

Centipede
Chestplate of Ineptitude (2 tier, max melee)
Vaward (for skill points, try max melee too)
Exhaustion (for more life steal)

Any 2 accessories, try max defense/max melee dmg Tisaun's Honour
Prowess (for skill points, this is optional if you're going for Tisaun's Honour. If not chosen, use Dupliblaze or Diamond Solar Bracelet)
Any necklace that boosts melee dmg/health/skill points (I use Diamond Solar Necklace)

And a good life steal Apocalypse
