My Quest Is Broken

Hi,

I've been stuck on the 'Lost souls' Quest for over a week, mainly because its broken for me.
In my quest book it states this," The face above the hall owns the nose that needs to be heavier to open the way" I stand on the nose and it does nothing.
The other problem is the NPC 'Grandon' is telling me different. "Throw something behind the map on the wall to uncover a piece of the code" I do as instructed and nothing happens.
My quest book and the NPC's don't match up. Is there anyone who could help me.

Im also stuck on the 'From the Mountains' Quest because I threw away the breathing helmet, and i need it for the quest, it wont let me go any further.

You need the Breathing Helmet II for From the Mountains. It can be bought at the armor merchant in Nesaak.
