Let me start off with a couple:

I once had like 3 lvl10 kiddos follow me throughout the whole of my 'whatever-the-hell-I-was-doing' session and then randomly leave.

While doing a quest in Troms I glitched out due to chunks not loading properly and I managed to see what was below me in a laboratory....This scared the hell outta me, some messed up Easter Egg creepy lion build with animated wings and googly eyes, Grian what the hell?!
