    what do you mean by neutral mobs?

    If you have lifesteal, it can be triggered by exploding, melee hits, and spell hits on any mob, including mobs with green names and blue names if thats what you are asking (I hit villagers to heal up).

    The chances of it being triggered are affected by the attack speed of your weapon, so its somewhat random.
