I am now showing this mushroom the world, I will be taking it all around Wynn. I cannot figure out what is with this end portal though.

 

It says something near Yahya, while entering Ragni and another place I believe

Well since we're posting random places that activates the Mushroom's dialogue, it says this at Maro Peaks:

Also, I already tried to fill in the empty slot with the Eye of Blood and Maro's Eye Jewel - neither worked.

This is a nice find but I don't think it's an entrance to Dern.

New province entrances are usually marked by some kind of hate and not a portal. In my opinion this portal might lead to a new area in the game world or something,dunno,but I don't think it's Dern.

There is one in Shattered Minds world, I found the tripwire (When we were hunting for ???) and a guy with who I was had a mushroom and I asked him to come, talking mushroom got a member of his familly in this world, the lime mushroom XD

Ice, it's been in the game for over 2 years....

The portal itself in a cave thats on the way from Ternaves to the Abandoned Mines. Its in front of the prison/mansion thingy you did a myth hunting video on. The cave entrance is kind of hidden and hard to see from the paths, so you're gonna have to search around a little.
