    Are you using any version of MC higher than 1.8.9? DerVillager is absolutely correct, 1.9 and 1.10 are horrible for horse movement - they are buggy. They can put you someplace far away from where you think you are. You can be standing with hostile mobs all around you, killing you, in the game, and you might not see any thing. You might also, as was suggested, fall through the floor into the void and die, and keep doing that, too. Go back to 1.8.9.
