So how often do loot chests respawn? Are they always at the same location? And what's a loot run and how do you do it?

Depends on the tier; tier 1s can spawn within a few minutes of them being opened and tier 4s take a while to appear again.
    Yes
A loot run is basically a route people set up that have many chests to loot (some chests higher in tier) in between each other. Doing it is as simple as following the route and looting the chests you see along it.

The chests do as far as i know respawn in ticks. That means that a T4 chest can be back before a T1 chest if youre lucky, but most likely the t1 is back earlier. But that doesnt matter, because the loot is best in chests that where untouched for a long time. Look for old servers to open loot chests. Im also releasing a guide SoonTM regarding loot running :)

A complete loot run often takes up to 30 minutes. However, a lot of servers will restart to prevent excessive lag in that server. That's when things get troublesome. If you rejoin a server and try to open loot chests, you need to wait for opening them, for a Tier 1 it is around 3 minutes to open it but for a Tier 4 the waiting time can take you u to 20 minutes just to wait for open a goddamn chest. (And mostly after the wait the content is useless lol)
It really depends on your luck. If you can manage to join an old server (servers that don't restart for more than half an hour) then your loot run can be pretty much safe and without obstacles.

Try to hop into servers with 40-43 people. Tier 4 chests won't spawn in low player servers (20-35 ish players), and hope for the server doesn't restart.
Unfortunately there maybe players doing loot run on the exact server and the same route as well, so you may find your chest missing.
Good luck finding a mythic though

Oh, I thought all servers reset on a daily basis.
How do you check which servers are old?

Often you'll find that "old" servers will typically have more players online than other servers. However, just be aware that someone else may also be lootrunning on that server.
