I’m to lazy to dig around for an image but is it Krolton's Remains from Beyond the Grave? Death’s isle, Grim trail: self explanitory, beast: Krolton, “tile”: Ilork the Npc guy (biggest stretch), inside given level range

of these, only envoy 1 & 2, mixed feelings, desperate metal, and ??? involve islands iirc
but what item could it be...
