I'd go with the first one since it has slightly more ehp as well as the fact that it has positive hpr. The second one has the benefit of not having to use mana steal tho.

 

Is there typically a downside to having to use mana steal?

 

Well generally if you use mana steal you need to use main attacks which means that you need to hit with main attacks and with mana regen you just get it passively, but mana steal does activate quicker than regen so y'know you pick
