just give inferno a bit of negative life-steal and it'll be balanced. Since you'll have to build with things like nether's scar or gaping cavity to account for the negative life-steal.

Mastery: Every 10 seconds, three of your skill points get +100, while the other two get -100.

Spring (Lament maybe?) - Inspiration: allies nearby get 2/4s mana regen.

Volcano - Burning clouds: Mana drains 2x faster when vanished, but grants you +120% damage boost instead of 80%.

 

Time to randomly lose armor pieces

 

It would work like consumable/weapon sp bonus, so no armor req change
