Does this means that, for example, if you have 2 CON and reach level 4, and you roll 6, you do :

You roll separately for each level.
So it'd work like this:
1st: 8 + 2
2nd: 1d8 + 2
3rd: 1d8 + 2
4th: 1d8 + 2
