As for four provinces; you are most certainly correct. Wynn, Gavel, and Fruma are the current recognized provinces, and Corkus is hoping to become the fourth. Hence, the rumours of a "supposed" or "future" fourth province coming from NPCs.

I always understood it as dern being the fourth province and the dark realm taking over dern

Dern has been a dark realm for as long as it's existed, according to current lore. It's not the Decay or the Corruption that took over, but the place where the endermen originate and a place of darkness, though possibly not evil.
