One seems small and insignificant, something you wouldn't otherwise think about, something quite meaningless

There are quests dealing with stuff like this, I would suggest you look for it in the area which the killer presided (I forget the name, but it had the guy who kept all their souls)

There are many options, but one I remember was not really a boss but a witch in gavel, who mind controlled the dude stopping you from passing, the witch is high on a ledge, but this doesn't sound like what we are looking for, but its an option

And not just that, you do indeed take a leap of faith in that quest.n(I should go get quest names for help)

Other one is probably in the Bigger Picture (pretty sure they used "perspective" in there), maybe at the same place with the small mushroom? (The one used in ???)
