First off, I notice that in your screenshot your Minecraft application is not responding. I'm assuming that after it recovers it disconnects you from the server. I'm also going to assume that the disconnect message is the generic "Connection Lost: Disconnected" message and not a Java message (such as the "Cannot connect to fallback server" join message).

I'm curious whether you played Wynncraft successfully without the modpack. Did the issue arise upon adding the modpack to your MC, or has it always existed ever since you began playing?

It's a possibility that your computer can't handle the modpack, so it freezes for 2 minutes and because of the freeze it disconnects you. Try removing the modpack and see if you can get into a Wynncraft world without it. If you can, the modpack may be creating the issue.

Another person just had the same issue: the game freezes upon entering a world, and after it recovers they are disconnected. My theory is that the modpack requires a high level of RAM or graphics in order to run. If there is not enough RAM or graphics power, the modpack will only cause the game to freeze.

You can attempt to allocate more RAM to Minecraft, but it depends on how much RAM your computer has overall. If you don't have much RAM, Minecraft cannot have much RAM allocated to it. Other than that, there isn't exactly a fix to it since it probably depends on your computer stats. If you can't get the modpack to run at all, the only thing you can do (unless you want to buy a new graphics card or computer) is to play without the modpack.
