There's no way to do that using Google Forms though and using some other not so well known website would deter people from voting. This is just a bit of fun after all though, so it won't be a disaster if someone votes for the same guy 5 times or something :)

 

You could always make 5 seperate forms

Also you could make it so 2nd place becomes mayor of a chosen city
And 3rd place idk....

 

But 5 forms won't prevent people from voting on the same guy on each forms.

I could do something like 2nd place being vice-president or something, good idea!
