    I'd finished the quest, and it didn't consume the Mythic Everlasting Pufferfish or given me Calamaro's Token at the end of the quest. Also, should I not have my Breathing Helmet I back?

    You don't need your Breathing Helmet I back because you now have a better version, that's perfectly fine. As for the quest item not being consumed I assume you mean you clicked on one of the guys and you didn't get it taken from your inventory or get the item reward? If that is the case it may be a glitch.
